7. Photocopy of Certificate of Completion (Incoming Grade 1, 7, 11)
8. Photocopy of NCAE Result (SHS Applicants)
9. Recommendation Form (Incoming Grade 2 to SHS)
10. Permanent Form / Form 137 (Incoming Grade 2 to SHS - FOR ENROLLMENT)
